COOK COUNTY CLERK'S OFFICE - BIRTH RECORD REQUEST FORM
A certified copy of a birth record is available at NO COST to persons born in Cook County who are verified as one of the following:
A)Homeless; B) Residents of shelters for victims of Domestic Abuse/Violence; C) A person released within the last 90 days
from the Illinois Department of
Corrections (IDOC) or the Cook County
Department of Corrections (CCDOC),
and/or D) A current inmate or
detainee who will be released within
the next 90 days from IDOC or CCDOC.
A status certification MUST be
completed by an agent or agency to
confirm that the requestor is
ELIGIBLE for a free birth
certificate.

To be eligible for the
homeless preference in admissions to the HACC’s
housing programs, applicants must meet the
following definition of homeless. Please check
the appropriate box and attach supporting
documentation from the list of recordkeeping
requirements. Documentation must be on an
agency’s letterhead. If this verification form is
being used as a self-certification, the applicant
must check the appropriate box and sign below.
Additionally, as part of the self-certification,
the applicant must participate in a homeless
assessment interview and provide a detailed
narrative description of homeless status as a
part of the eligibility process.

Teen Living Programs strives to create hope and opportunity for youth who are homeless by assisting them to permanently leave the streets, secure stable housing, and build self-sufficient, satisfying lives. Also offers residential programs. Has a health clinic on-site, performs career aptitudes and assistance with employment, tutoring and referrals for GED programs and alternative schools.

CEDA's LIHEAP programs offer heat and electricity assistance through the DVP and PIPP programs. DVP issues a one-time benefit applied directly to your utility accounts. PIPP enrolls you in a budget bill program that provides a monthly benefit toward your bill and offers the opportunity for past due balance forgiveness.

Illinois Collaboration on Youth
The Homeless Youth program ensures that youth (ages 14 to 23) stay safe, stable, and self-sufficient when their housing situations are dangerous and unreliable. The program provides safe and stable housing, education and employment services, and the life skills necessary to become self-sufficient, ultimately reducing the number of homeless youth in Illinois.
